I am a general practitioner in the internal medicine department cooperating with a medical institution outside the prefecture (Aomori) . As a member of the international working group called GRADE, I established an HP/Blog to offer information about the GRADE system and facilitate its spread in Japan. The risk of cardiovascular events or the mortality rate for five years can be predicted using the JLIT or the Koriyama point, both of which are prospective studies of Japanese cohorts.
GRADE system:
Clinical Practice Guidelines (CPG) offer recommendations for the management of typical patients. Such clinical decisions involve weighing the desirable and undesirable effects of a given course of action. In order to help clinicians make evidence-based medical decisions, guideline developers often grade the strength of their recommendations. Prior grading systems had many disadvantages, including the lack of differentiation between quality of evidence and strength of a recommendation, the lack of transparency in emitting a judgment of the values and preferences underlying the recommendations.
In addition, the existence of many, often scientifically outdated, grading systems has created confusion among guideline developers as well as users.
