ilvl is the level of the spawned item. qlvl is the quality level of each base item (see the base armor/weapon section).
If qlvl > ilvl then qlvl determines the affixes, else ilvl determines. Upper items can have good affixes even if the ilvl is low.
If ilvl (read qlvl, if higher) <=66, the level of available affixes (alvl) is ilvl / 2, so if ilvl = 66 then alvl = 33.
If ilvl > 66 alvl increases by 2, so if ilvl = 67 then alvl = 35, if ilvl = 68 then alvl = 37.
Cruel is a level 51 affix, so base weapons of qlvl >= 76 always have a chance to be cruel.
Some items like wands and circlets have magic level (mlvl).
If mlvl > 0 then always alvl = ilvl + mlvl (ilvl is not divided by 2), so even 1 mlvl makes much difference.
